Radnja je smeštena u Rengkoku, mračnom i futurističkom megalopolisu sa misijom glavnog protagoniste da spase svog brata otetog od strane Heaven konglomerata. U pomoć pristiže misteriozni haker kao i arsenal šarenolikog oružja. Ruiner takođe poseduje odličnu vizuelnu lepotu, i lepo možemo videti da su kreatori bili inspirisani anime naslovima, poput Ghost in the Shell. Iza igre stoje veterani industrije, koji su ogromno iskustvo stekli u radu na The Witcher serijalu, zajedno sa izdavačem koji nam je doneo odlični Hotline Miami. Čitavo sajbepank okruženje je izrađeno u Unreal 4 endžinu što Ruiner čini još lepšim.
